Clinical Innovation Guidelines
Amount: $1000
Types of Projects: Projects that integration of current best clinical evidence with clinical expertise to guide practice. Examples of projects you might want to consider submitting include - quality improvement projects at the organization or unit level, benchmarking projects, research utilization projects, case-management projects, clinical practice guideline projects.
Submissions:
· Applications will be limited to one per lead investigator.
· Faxed submissions will not be accepted.
· Maximum of 10 double-spaced, typewritten pages, using 12-point font and one-inch margins
Title (Page 1)
· Title page should include title of the clinical innovation project, names of all project members, contact information for lead investigator(s), and three to five words that reflect the project area.
· Include and email address for the lead investigator(s).
Abstract (Page 2)
An abstract of less than 150 words; include an overview of the clinical innovation strategy implemented and the outcomes and/or results obtained (or anticipated).
Part I. Body of Application (3 pages minimum to 10 pages maximum)
a. Clinical Practice Problem
§ Briefly discuss the clinical problem and how it is presented
b. Clinical Evidence
§ Describe the type of clinical evidence used, i.e., hand search of published literature, searches of electronic databases, patient registry data, searches of unpublished data, etc.
§ Describe the search strategy, e.g. lists of journals scanned, keywords, database sources, etc.
c. Method used to review evidence
§ Identify the methods used to evaluate evidence
§ Identify the rating scheme used, if any
d. Strategy for Improvement
§ Outline the strategy
e. Implementation of Strategy
§ Outline the implementation plan
f. Evaluation Methods
g. Practice Outcomes/Results
§ Identify the practice outcomes that you will improve (or were improved).
h. Lessons Learned
§ Include this section for projects already completed
Part II. Reference List, Appendices, and Timeline
§ Reference List
§ Use APA format to reference citations used in the text
§ Appendices
§ Letter(s) of support from clinical agency or practice site(s) at which the project was implemented
§ Timeline
